Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

• Medicals for pre-employment, sports, driving requirements and adoption/fostering
• (HGV,PSV,AH1&AH2 etc)
• Insurance claim forms
• Prescriptions for taking medication abroad
• Private sick notes
• To Whom It May Concern letter
• Forms for collection (see below)

The fees charged are based on the British Medical Association(BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

If you leave a form which needs completing by a doctor, it will only be ready for collection after ten clear working days, please be mindful that this may differ dependant on the paperwork and the availability of the GP's as some doctors only work part-time. Payments must be made by card only, no cash accepted.

For other forms of paperwork which incur a charge ie: holiday cancellation, insurance etc. Payment must be made upfront.

Note: forms maybe completed by a different doctor than requested.
